Alumni/Former Student Transcript Request

Graduates and former students should use this form when requesting the release of their high school transcript to a third party.  Please follow the instructions provided on the form.  Please note that transcript requests must be submitted at least 48 hours (i.e. two full school days) in advance of when the transcript is needed.
